You are one of the\u00a0leading advocates for the diabetic community.\u00a0In 2007, you wrote an open <u><a onclick=\"javascript:pageTracker._trackPageview('\/outgoing\/www.healthline.com\/health\/diabetesmine\/innovation\/open-letter-steve-jobs#1');\"  href=\"https:\/\/www.healthline.com\/health\/diabetesmine\/innovation\/open-letter-steve-jobs#1\" target=\"_blank\">letter to Steve Jobs<\/a>\u00a0<\/u>that went viral, asking\u00a0Apple to apply the same design skills to medical devices that Apple devoted to its consumer products. What happened since then?<\/strong><\/p>\n<p><strong>Amy Tenderich:\u00a0<\/strong>There has been a true Revolution in Diabetes Technology and the \u201cconsumerization\u201d of medical devices in general\u2026 and I\u2019m thrilled to be part of it!\u00a0As I laid out in my <a onclick=\"javascript:pageTracker._trackPageview('\/outgoing\/www.healthline.com\/diabetesmine\/open-letter-steve-jobs-10-years-later-milestones-patient-innovation#1');\"  href=\"https:\/\/www.healthline.com\/diabetesmine\/open-letter-steve-jobs-10-years-later-milestones-patient-innovation#1\">\u201c10 Years Later\u201d post<\/a>, the biggest milestones are:<\/p>\n<ul>\n<li>Upsurge of patient involvement in innovation\/design<\/li>\n<li>Shift to data-driven disease care that increasingly prioritizes Interoperability of devices and data<\/li>\n<li>US FDA forging a path for open, candid interaction between the regulatory agency and the patient community \u2013 which we had a hand in (very exciting!)<\/li>\n<li>Consumer giants like Apple, Google, Microsoft, Samsung and others getting involved in healthcare, and diabetes specifically &#8212; which changes the landscape and mindset for products and services<\/li>\n<\/ul>\n<p><strong>Q2. At that time you wrote that the devices the diabetic community had to live with were \u201cstuck in a bygone era\u201d, created in an \u201cengineering-driven, physician-centered bubble.\u201d \u00a0How is the situation now? <\/strong><\/p>\n<p><strong>Amy Tenderich:\u00a0<\/strong>With the help of our prodding, medical products are now designed to be more compact, more comfortable, more aesthetic and more personalizable than ever before. In other words, they\u2019re now keeping pace with consumer tech products.<\/p>\n<p>For examples, see the <a onclick=\"javascript:pageTracker._trackPageview('\/outgoing\/www.tandemdiabetes.com\/products\/t-slim-x2-insulin-pump');\"  href=\"https:\/\/www.tandemdiabetes.com\/products\/t-slim-x2-insulin-pump\">Tandem t:slim insulin pump<\/a> and the <a onclick=\"javascript:pageTracker._trackPageview('\/outgoing\/onedrop.today\/?gclid=EAIaIQobChMIq9vqlvP-5QIVWxitBh0vawsHEAAYASAAEgJJQfD_BwE');\"  href=\"https:\/\/onedrop.today\/?gclid=EAIaIQobChMIq9vqlvP-5QIVWxitBh0vawsHEAAYASAAEgJJQfD_BwE\">One Drop glucose meter<\/a> \u2013 which both resemble Apple products \u2013 the <a onclick=\"javascript:pageTracker._trackPageview('\/outgoing\/www.quellrelief.com\/');\"  href=\"https:\/\/www.quellrelief.com\/\">Quell<\/a> pain relief solution, and the dynamic, fun-to-use <a onclick=\"javascript:pageTracker._trackPageview('\/outgoing\/mysugr.com\/en-us\/');\"  href=\"https:\/\/mysugr.com\/en-us\/\">MySugr diabetes data app<\/a>.<\/p>\n<p><strong>Q3. Why is it so hard to\u00a0bring the tech and pharma worlds together?<\/strong><\/p>\n<p><strong>Amy Tenderich:\u00a0<\/strong>Good question! Check out the 2012 Altantic article titled, \u201c<a onclick=\"javascript:pageTracker._trackPageview('\/outgoing\/www.theatlantic.com\/technology\/archive\/2012\/03\/the-reason-silicon-valley-hasnt-built-a-good-health-app\/254229\/');\"  href=\"https:\/\/www.theatlantic.com\/technology\/archive\/2012\/03\/the-reason-silicon-valley-hasnt-built-a-good-health-app\/254229\/\">The Reason Silicon Valley Hasn\u2019t Built a Good Health App.<\/a>\u201d It basically outlines how technology companies tend to focus on the tech itself, without understanding the real-world use case.<br \/>\nAlso, tech companies tend to develop and iterate at breakneck speed, whereas the healthcare world \u2013 especially big legacy pharma companies \u2013 are burdened by loads of regulations and historically moved at a glacial pace.<\/p>\n<p>The good thing is, these two worlds are inching closer together as:<\/p>\n<ul>\n<li>Pharma companies are by necessity transforming themselves into digital organizations that deal in software and innovate more rapidly, and<\/li>\n<\/ul>\n<ul>\n<li>Tech companies are \u201cgetting religion\u201d on understanding the real-world aspects of people\u2019s health and disease care.<\/li>\n<\/ul>\n<p><strong>Q4. Who are the key\u00a0diabetes \u201cstakeholders\u201d?<\/strong><\/p>\n<p><strong>Amy Tenderich:\u00a0<\/strong>Patients and caregivers, first and foremost, as the people literally \u201cliving this illness.\u201d Then of course: Pharma and Medtech companies, FDA regulators, clinicians, researchers, other healthcare providers (eg. Certified Diabetes Educators), non-profit advocacy groups, health data platform and app developers, and healthcare designers.<\/p>\n<p><strong>Q5.\u00a0Artificial Intelligence and Machine Learning (ML) are becoming widely discussed and employed in the diabetes tech world. What is your take on this?<\/strong><\/p>\n<p><strong>Amy Tenderich:\u00a0<\/strong>Indeed, AI\/ML appear to be the wave of the future. All data-driven tools for diabetes care \u2013 including new <a onclick=\"javascript:pageTracker._trackPageview('\/outgoing\/www.healthline.com\/diabetesmine\/artificial-pancreas-what-you-should-know');\"  href=\"https:\/\/www.healthline.com\/diabetesmine\/artificial-pancreas-what-you-should-know\">Artificial Pancreas tech<\/a> on the horizon \u2013 is based on these advanced computing techniques.<\/p>\n<p><strong>Q6. When using AI for diabetes: what are the main new\u00a0regulatory and ethical issues that need to be faced?<\/strong><\/p>\n<p><strong>Amy Tenderich:\u00a0<\/strong>We were fortunate to have <a onclick=\"javascript:pageTracker._trackPageview('\/outgoing\/www.linkedin.com\/in\/wevans\/');\"  href=\"https:\/\/www.linkedin.com\/in\/wevans\/\" target=\"_blank\">Bill Evans<\/a>, Managing Director of Rock Health, <a onclick=\"javascript:pageTracker._trackPageview('\/outgoing\/www.slideshare.net\/amytenderich\/bill-evans-rock-heatlh-on-machine-learning-diabetesmine-university');\"  href=\"https:\/\/www.slideshare.net\/amytenderich\/bill-evans-rock-heatlh-on-machine-learning-diabetesmine-university\">present on this topic<\/a> at our 2018 DiabetesMine Innovation Summit.<\/p>\n<p>His slide on \u201c<em>Seven Threats to AI<\/em>\u201d laid out the following:<\/p>\n<p><em>AI\/ML STARTUPS AND PRACTITIONERS :<\/em><\/p>\n<ul>\n<li>Over-focusing on \u201cshiny objects\u201d vs. the UX and business value.<\/li>\n<li>Smart algorithms are being trained on dumb and dirty data.<\/li>\n<li>Practitioners are building \u201cblack boxes\u201d even they can\u2019t understand.<\/li>\n<\/ul>\n<p><em>ENTERPRISE LEADERS :<\/em><\/p>\n<ul>\n<li>Though they\u2019re the key customers, most enterprise organizations don\u2019t know where to begin.<\/li>\n<li>Major incumbents possess\u2014but fail to capitalize on\u2014the most valuable commodity: Data.<\/li>\n<\/ul>\n<p><em>INVESTORS<\/em> : Hype allows some companies to masquerade as \u201cAI\u201d companies.<\/p>\n<p><em>REGULATORS<\/em> : Regulation of AI\/ML still needs to come into focus.<\/p>\n<p>Evans and Rock Health have actually been instrumental in helping the US FDA decide how to approach regulation of AI and Machine Learning in Healthcare. Their work focuses on gaining consensus around \u201cground truth data.\u201d You can read all about it and even weigh in <a onclick=\"javascript:pageTracker._trackPageview('\/outgoing\/rockhealth.com\/how-should-the-fda-approach-the-regulation-of-ai-and-machine-learning-in-healthcare\/');\"  href=\"https:\/\/rockhealth.com\/how-should-the-fda-approach-the-regulation-of-ai-and-machine-learning-in-healthcare\/\">here<\/a>.<\/p>\n<p><strong>Q7. Which do you care more about: Accelerating medical advances or protecting data rights?<\/strong><\/p>\n<p><strong>Amy Tenderich: \u00a0<\/strong>The hope is that these are not mutually exclusive. But if you ask people in the Diabetes Community, I believe they would almost always prioritize accelerating medical advances.<\/p>\n<p>That\u2019s because type 1 diabetes is a potentially deadly disease that requires 24\/7 effort just to stay out of the hospital. Data privacy seems a small trade-off for many people to get better tools that aid in our survival and reduce the disease burden.<\/p>\n<p><strong>Q8.\u00a0Many in the Diabetes Community are turning to DIY tech to create their own data-sharing tools and so-called Automated Insulin Delivery (or &#8220;closed loop&#8221;) systems.\u00a0 Can you please explain what this means? Is it legal?<\/strong><\/p>\n<p><strong>Amy Tenderich: \u00a0<\/strong>I\u2019m proud to say that we at DiabetesMine helped launch the #WeAreNotWaiting community rallying around this DIY tech.<\/p>\n<p>That is, the now-hashtag \u201cWe Are Not Waiting\u201d was the result of a group discussion at the very first<u> <a onclick=\"javascript:pageTracker._trackPageview('\/outgoing\/www.healthline.com\/health\/diabetesmine\/innovation\/d-data-exchange#1');\"  href=\"https:\/\/www.healthline.com\/health\/diabetesmine\/innovation\/d-data-exchange#1\" target=\"_blank\">DiabetesMine D-Data ExChange<\/a><\/u> technology forum in November 2013 at Stanford University. We gathered some of the early tech-savvy patient pioneers who were beginning to reverse-engineer existing products and develop their own platforms, apps and cloud-based solutions to help people with diabetes better utilize devices and health data for improved outcomes.<\/p>\n<p>Today, there is a vibrant community of thousands of patients using (and iterating on) their own homemade \u201cclosed loop systems\u201d around the world. These systems connect a continuous glucose monitor (CGM) with an insulin pump via a sophisticated algorithm that essentially automates insulin dosing.\u00a0Current systems do still require some user intervention (so the loop is not completely \u201cclosed\u201d), but they greatly improve overall glucose control and quality of life for patients.<\/p>\n<p>These DIY systems have not been approved by FDA for safety and effectiveness, but they are by no means illegal. In fact, the results have been so powerful that no less than 6 companies are seeking FDA approval for commercial systems with the same functionality. And one popular DIY model called Loop has been taken up by an \u00ad\u00ad\u00ad\u00adoutfit called Tidepool for <a onclick=\"javascript:pageTracker._trackPageview('\/outgoing\/www.healthline.com\/diabetesmine\/newsflash-tidepool-loop');\"  href=\"https:\/\/www.healthline.com\/diabetesmine\/newsflash-tidepool-loop\">conversion into a commercial, FDA-scrutinized product<\/a>.<\/p>\n<p><strong>Q9 Is it possible to use Social Media for real Health Impact?<\/strong><\/p>\n<p><strong>Amy Tenderich: \u00a0<\/strong>Most certainly, yes. There is a growing body of evidence showing real-world impact on improved health outcomes.\u00a0See for example this recent <a onclick=\"javascript:pageTracker._trackPageview('\/outgoing\/www.evariant.com\/blog\/the-evolving-role-of-social-media-in-healthcare');\"  href=\"https:\/\/www.evariant.com\/blog\/the-evolving-role-of-social-media-in-healthcare\">eVariant article<\/a> that cites the benefits of patient-powered research networks, and states, \u201cThere\u2019s no question that patients use the Internet to take control of their own health.\u201d<\/p>\n<p>See also, original research from our DiabetesMine team, published in the <a onclick=\"javascript:pageTracker._trackPageview('\/outgoing\/www.ncbi.nlm.nih.gov\/pubmed\/30477335');\"  href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/30477335\">Journal of Diabetes Science and Technology (Nov 2018)<\/a>: \u201cFindings indicate that social media provides a significant source not only of moral support and camaraderie, but also critical education on thriving with diabetes. Importantly, we observed strong evidence of peer influence on patients&#8217; therapy and diabetes technology purchasing decisions.\u201d<\/p>\n<p><strong>Q10 What is the FDA mHealth Pre-certification Program? \u00a0and what it Means for Diabetes?<\/strong><\/p>\n<p><strong>Amy Tenderich: \u00a0<\/strong>This is the FDA\u2019s revolutionary move to change how it reviews mobile apps and digital health software to accelerate the regulatory process and get these products out there for people to start using ASAP.<\/p>\n<p>The agency announced its Pre-Certification for Software Pilot Program in July 2017.
